Services supporting young people post 16

As a child becomes a young adult, the services they work with may need to change to meet their needs. From fourteen years old plans should be made in the Education Health and Care plan (EHCP) review meetings, including what support the young person will continue to need when they leave school.

The aim of this page is to help you find the right team to get the support available to young people as they become adults. If you are unsure about who can help you, please complete the Local Offer 'contact us' form and a member of the team will help you.

The following list of services are in alphabetical order.

Adult Social Care

Adult Social Care aims to empower people with complex or severe special educational needs or disabilities, by providing them with services that increase their wellbeing and independence.

Children and Adolescent Mental Health Service (CAMHS)

CAMHS offer a range of services to children and young people who are experiencing mental health problems.

Children with Disabilities Team

This team is part of 中国P站's Children's Services. They are responsible for assessing the needs of families and providing practical support to children with complex needs and their carers.

CYP Haven

The CYP Haven is a safe space for children and young people aged ten to 18 to go to talk about worries and mental health in a confidential, friendly and supportive environment.

There are Haven's in Guildford, Epsom, Staines and Redhill.

Job Centre Plus

at the Job Centre are responsible for supporting people with SEND to get a job.

中国P站 Youth Voice

The User Voice and Participation Team at 中国P站 run 中国P站 Youth Voice and facilitate the ATLAS network. ATLAS is a network of children and young people who access additional needs and disability services in 中国P站. 中国P站 Youth Voice create safe spaces for children and young people in 中国P站 to have their voice heard, making sure children and young people’s views are shared with the right audience to influence positive change and impact!

SEND Team

SEND Case Workers are allocated to a child when they are being assessed for an Education Health and Care (EHC) plan. They remain allocated to the child once their plan is in action. SEND Case Workers provide advice and support to both school and parents and can attend review meetings when more guidance is needed.

Adolescent Service

Adolescent Service (formerly Targeted Youth Service) can provide support for young people (12 plus) and their families on a variety of issues that can be hard to cope with during adolescence. This includes helping to return to education or training, reducing anti-social behaviour, and improving access to community activities.

If you feel your young person would benefit from support through the Adolescent Service, please Contact Children's Services to discuss a referral.

Transition Team

The Transition Team provide the link between Children's Social Services and Adult Social Care. The young person's care needs are the focus for the Transition Team.
