The reasons why we use your personal data

The Public Health team works across a number of key areas of health improvement and protection for the population of ÖйúPÕ¾. Your personal data will be collected as part of your participation in the Changing Futures programme. We may use your personal data to contact you throughout your participation in the programme.

Bridge the Gap Service provider scheme. This involves the ÖйúPÕ¾ Adults Matter team working with the various Bridge the Gap Providers to improve the lives of people facing multiple disadvantages.

National Government Changing Futures project provision. This involves ÖйúPÕ¾ (Public Services Reform and Health Wellbeing and Adult Social Care Services respectively) hosting the relationship on behalf of the Ministry of Housing, Communities and Local Government (MHCLG) with the client. MHCLG, as the data controller, are liaising with other government departments and agencies as part of the Changing Futures Project. See the and the for further information.

How we use your data

Personal information as processed by the Service and also the Bridge the Gap providers is used as follows:

• Service delivery
• Service improvement and planning
• Research
• Statistical analysis and reporting
• Safeguarding

As part of the Service offered by the MHCLG in its remit as the data controller (Changing Futures project) the personal information is used as follows:

• to stabilise and then improve the life situation of adults who face multiple disadvantage
• to transform local services to provide a person-centred approach and to reduce crisis demand.
• to test a different approach to funding, accountability and engagement between local commissioners and services, and between central government and local areas. See .

What information do we collect?

The Bridge the Gap Service and the Changing Futures project will collect various personal information depending on the client and their circumstances including the following:

• Your name
• Mobile and telephone number
• Email address
• Home address including postcode
• Date of birth
• Gender identity
• Sexual orientation
• Religion
• Disability status
• Marital status
• National Insurance number
• NHS number
• Health, safety and well being
• Criminal offences
• Personal identifier (pseudonymised number if applicable)
• Housing situation
• Use of Health Care Services
• Benefits received
• Client referral information

How we are allowed to use your personal data

The MHCLG has advised the Council as follows concerning the processing of their personal information:

• Article 6(1)(e) of the UK General Data Protection Regulations (GDPR) which states that, as a government department, MHCLG may process personal data as necessary for our work in the public interest.
• Article 6(1)(e) - the processing is necessary for MHCLG to perform a task in the public interest or in the exercise of official authority. Section 8(d) of the Data Protection Act (DPA) 2018 makes provision that this includes processing that is necessary for a function of the Crown, a Minister of the Crown or a government department. This function is funding projects aimed at supporting those individuals experiencing multiple disadvantages.
• In order to rely on Article 6(1)(e), Article 6(3) requires there to be a separate legal basis laid down in UK law to justify the processing. MHCLG is relying on its common law powers to justify this processing. The Secretary of State for Housing, Communities and Local Government relies on the common law powers of the Crown as the separate legal basis for this processing.

The following is the UK GDPR lawful basis under which the Fund will process the personal data:

• Article 6(1)(e) – the processing is necessary for the Fund to perform a task in the public interest or in the exercise of official authority. The Fund is a statutory corporation, created by the National Lottery etc. Act 1993, all its powers and functions are statutory and come from the National Lottery etc. Act 1993. When the Fund processes personal data in connection with applications and management of lottery grants, it is exercising official authority vested in it by the National Lottery etc. Act 1993.

Consent

As part of the Bridge the Gap process, we will explain to you what we are asking you to agree to and why. If we have consent to use your personal data, you have the right to remove it at any time. If you want to remove your consent, please contact us at the following email address btg.referrals@surreycc.gov.uk and we will deal with your request.

In some circumstances, Article 9(2) (g) may apply to the processing which is necessary for the reasons of substantial interest. In these instances the Council will be meeting the following substantial public interest test condition as established in Schedule 1 Part 2 section 16 of the DPA 2018 support for individuals with a particular disability or medical condition.

In respect of processing of any criminal convictions, allegations, investigations and proceedings then the additional condition as specified in Schedule 1 Part 3 section 29 of DPA 2018 will apply.

The Bridge the Gap process relies initially on verbal consent (dependent on client condition) and then subsequently written explicit consent.

Who we share your personal data with

As part of the Bridge the Gap provision the information will be shared as follows:

• Between the ÖйúPÕ¾ Adults Matter team and Public Health and the appropriate Bridge the Gap providers and local community agencies
• Seetec Limited as part of assessment of project effectiveness

As part of the Changing Futures project, the Council is hosting the relationship on behalf of the MHCLG, as data controller, so the information will be shared within the Bridge the Gap process and then onwardly including as follows:

• Areas outside the programme to share learning from the programme and provide a national network.

Retention of data

Bridge the Gap information will be retained for 6 years

Data will be retained in accordance with our countywide data retention schedule.
